Slightly Odd Motherboard Standoff Placement

No, no need to drill any "custom" holes.

The 1st hole (top hole that you circled) is at standard location for all / most cases.

The second hole is for extra case compatibility.

You don't need to use both of those stand-off holes / locations.

well what is your case? are you placeing this board in the case like this because this is sideway. the IO (top right in this pic) is usually on the top left and towards the back of the case. and at 235mm or 9,25in. it seems like you have a micro ATX board and not a full ATX but i am just guessing on this.

It's common for ATX boards nowadays to be narrower. 

The hole on your board closest to the grey SATA ports isn't part of the ATX standard and you can simply ignore it. Your board will probably flex quite a bit when plugging in the 24-pin tho. So be prepared for that, by either ignoring it (yup, I'm serious...) or supporting the back of the board somehow.

 

My AsRock X570 board is the exact same, but doesn't have that hole near the SATA ports.
Imo it's pretty silly that boards aren't the full ATX width anymore.

you can screw and standoff in the bottom hole but not attach it to the case.
