Going from low to medium is a step up, but going from medium to high or beyond usually is not as big of a step up. Often times it's only the shadows and anti-aliasing that get significantly better.

The texture files usually stay the same size.

But I think it will really depend on the game and situation, so we really can't answer your question without some context (game, settings you had, settings you have now, what you think you're experiencing)

Depends on the game.

For what it's worth, the Forza games only start introducing more minor quality differences after high settings. They're predominately built for consoles, so even at their highest settings, they won't look massively better than at high, which is the target setting mark for the Xbox One games.

Arent these all that there is for major settings? Textures depends on monitor resolution, so ultra textures at 1080p may look the same as high

From low to medium/high it is very noticeable in some games, and in some settings. The water in tw3 for example, its way better on high than on low or medium. It depends a lot on the game and the setting, but a lot of settings usually have minir differences and a lot of impact on performance.

As for ultra, its just a "tank performance, change nothing else" button. Never use ultra, unless your fps is maxed out anyways... Ultra is just stupid.

Ultra is stupid. ALWAYS.
