Micro SD Card won't format to FAT32, Help!

I currently have a Micro SD card that work perfectly fine with my Samsung device, as soon as i go to format it to FAT32 to use it for another purpose it won't let me. I'll attach screen shot showing you some methods of the many that i have tried.

If you cant Diskpart/ Format via GUI then I would suggest its time to get a new SD card :(

does the SD card have a lock on it?

it should be unlocked

The SD card does have a physical lock button on it but it's turn off. I made sure of that ;) ... And what doesn't make sense is that i can put it in my Samsung phone and it works perfectly. As well as this is not the only SD card that it's happened to. I would like to find a fix for it if its going to continue to happen.

it says it has an IO error..that basically means the computer isn't reading the card properly...try it on another computer

also..try formating it in linux

Try my laptop and my gf laptop, neither worked. and tried a bootable usb with linux mint, tried formatting in with Gparted and would not work either. Ill probably wive linux another try in case i missed something since I'm not to familiar with linux. 

 

Was playing around with this for multiple hours yesterday and nothing. and it just doesn't make since because it works perfectly when put into my phone... one of the things i found online was to use a program called "M3 RAW drive recovery" since in the last photo i posted it shows the SD card is in RAW format...but seems there's a cost to that and I'm not willing to pay.

Try a Live bootup cd with Ubuntu and use Disks to format it.
