What kind of port is this?

I'm not sure what the official name for that is, I only know it as a "box header" As for what it's used... well, that depends entirely on what it's on. It can be used literally for anything.

looks like a RS_232 connector. But there's a lot proprietary stuff out there. What board (Vendor/Model/Revision) is it?

It's inside an old Dell optiplex 755
