Helping my dad with iPhone issues...

So my dad bought the iPhone 7 when it came out. Over the past 2 months the phone has just randomly changed from the text, notifications, and ringtone that he has selected. Sometimes it uses the ones that he has selected, but more often than not, it will use the default ones.

 

He is not technological illiterate (he has taught me almost everything that I know), so he has tried the obvious steps: restarting the phone, resetting and restoring from a backup, changing tones. He always keeps it on the latest stable (so he isn't in the iOS Beta program) iOS build.

 

Any ideas as to why this may be happening and how we can go about fixing it?

 

And before everyone says: "Solution: Switch to Android", him and my step-mom (she has a 5S) are the only ones in my immediate family that have iPhones, I have an LG G5, and I'm thinking about upgrading to the G6 when it releases.
