looks either like a survey or a warranty. 

I can read Japanese and Japanese and Chinese use some of the same kanji.

1 of the kanji is the same kanji used in warranty in japanese.

I also see the kanji for 1 month.

Warranty Card

Please keep this warranty card and a proof of purchase, you will need those if you need to repair your product, if you fail to show both your warranty will be voided.  The duration of your warranty will be determined by the date of purchase on your warranty card.

 

Service Rules

2. Product malfunction under normal usage within 1 month can be completely replaced.

So basically you probably won't get any service. Next time buy things made in Taiwan. Better quality, better service. Just look at Dennis Liao, he was made in Taiwan, such quality.
