I enjoyed Anna quite a bit, but I wouldn't really call it realistic.

System Shock 2 might not be scary, but it is quite suspenseful.

 

If you have a Xbox360 console, then both Condemmed games are worth playing

If scares are what you're after, I'd try a game like Amnesia: Dark Descent.

or fear ;)  cute alma will scare you

until dawn got some nice moments too

I recommend real life...

Random starts and starting stats, possible random deaths... you can get killed in 1 hit..

always online, excellent graphics... heck, I'm so addicted right now, I can't even quit if I wanted to...

the game environment is pretty dynamic... so dynamic that it can kill you...

not to mention all the minigames...

oh.. and it's huge too... so huge that no single player has been able to explore the entire map...

I think Alien: Isolation is the only actual horror game I've ever played, but it's definitely scary, and it's definitely immersive (if that's what 'realistic' means to you).

 

SOMA looks like something I'd like and I do plan to get it eventually. I have way too much to play right now.

The most realistic ? Get ARMA 3(or ARMA 2) and get into military multiplayer missions. War is the the scariest thing I can think of. And getting into ambush with your squad from unknown location can be really scary. And of course you have one life. Once you die you're dead until you organize a new mission with people.
