Could someone please help me identify this part?

Looks like a 4 port PCIe NIC or something

More pics would be helpful.

you don't have to attach links, you can insert images directly to your LTT posts.

also we can't even see the ports with the pics you provided so to us it's mostly just a PCB with a PCI connector on it.

I WILL find your ITX build thread, and I WILL recommend the SIlverstone Sugo SG13B

Primary PC:

i7 8086k - EVGA Z370 Classified K - G.Skill Trident Z RGB - WD SN750 - Jedi Order Titan Xp - Hyper 212 Black (with RGB Riing flair) - EVGA G3 650W - dual booting Windows 10 and Linux - Black and green theme, Razer brainwashed me.

Draws 400 watts under max load, for reference.

How many watts do I needATX 3.0 & PCIe 5.0 spec, PSU misconceptions, protections explainedgroup reg is bad

I think those are RJ11 ports, not RJ45, so old phone cables. @Lurick almost had it.

Not really useful anymore. Do you even have a PC with PCI in it? PCIe has taken over, and judging by the rainbow barf keyboard, you've likely got something at least semi modern.

RJ11 was a similar connector to RJ45. Really good for long range data connections with low loss. This is the old equivalent to a gigabit nic with a bunch of Ethernet ports.
