The guy who played threedog has tweeted two times concerning the game. 

 

It is infact in development, it will be set in boston ( recent rumors confirm this ) and bethesda has outright stated they are actively in development of an open world game. It's fallout 4!!!!

Bethesda hasn't announced that there are even working on the game... So no one even knows if they are, and if they are how far they are.

Yeah, Fallout 4 isn't confirmed by a long shot. Bethesda has never mentioned Fallout 4 at all. For all we know they could be making another game right now. 

However, Bethesda does know that many people are waiting for a new fallout. Also, they completely bought the rights to the Fallout franchise, so they are defiantly planning on making more than just Fallout 3.
