Mine is on a wordpad document in an external truecrypt encrypted HDD. I also have versioning equations for most of the more "sensitive" passwords.

A water-cooled mid-tier gaming PC.

Easy, i use the same Email "obviously" and password for every single account i have with some changes on capitalisation on the password
