Depends on the game.  I'm running a VG248QE, so for games that are reflex intensive, I generally dial in the settings so I get ~180-200fps (not too hard with dual 290s) and then use RTSS to lock the max framerate at ~146.  Generally keeps me from dropping under 144 in all but the most intensive of scenes. 

However, if it's something casual (like hearthstone) I'll use v-sync because why not?

some of the games I play such as Shadow of Mordor, CS:GO, Borderlands 2 have tolerable screen tearing, so I'll leave V-Sync off for those.  However, games like Bioshock Infinite have screen tearing that's so bad that it's hard to appreciate any of the graphics.

I have close to never experienced noticing tearing or stuttering, so not using V-sync because I see no use for it.

Also, having a 120hz monitor helps

I rather appreciate v-sync and will use g-sync as soon as I can get monitor that supports it within my budget. In fairness however, I cannot use v-sync with mmo's and never recommend anyone try to because the frame rates are always extremely unstable (read stable 90 fps +/-70 fps) which can randomly lead to massive input lag at the wrong moments.

I prefer to use it when I see tearing.

If I don't see tearing, I don't turn it on.

Running minecraft at 600+ fps is healthy for graphics cards :D
