Graphics card issue. It's likely dying or an extremely unstable OC.

If you have not overclocked I recommend sending it for RMA if it's still under the warranty. If not, you can try underclocking it, especially the VRAM. May not work though.

Your card may be running so hot that the VRAMS are crashing. Has it ever been cleaned?

Looking at how dusty the cooler is... it very well could be. Just because it didn't run hot in the past doesn't mean it won't run hot once dust starts to gunk up the cooler.

Technically if it was from overheating, it should be fine after letting it cool and booting up, but the moment it gets to hot it will crash again (and heat kills silicon so doing this for funzies isnt ideal).

i would say its dead, i played vr games, recorded with high bitrate, used 7+G on mc modpacks with 200 mods, exported with premiere pro at 1080p HD

You can do that all day for years... But before it gets that old you do want to take it apart and replace the thermal paste and pads at some point to increase your chances if you intend to keep using it.

Those are tasks GPUs are built to do, not things that would kill them. Running 24/7 FurMark on an old GPU like this that wouldn't intelligently throttle while stuffing the cooler full of dust would be one of the few ways to actually kill a GPU just by running stuff on it. 

It's likely just overheating VRAM like others have mentioned, giving it a clean and repaste is worth a shot.
