Need help on my hard drive

I have a laptop which is acer aspire 5 mx150 it 8th gen and 1tb hard drive 5100RPM but it is always in 100% usage and is very slow plzz help. I need to know weather its a software issue or hardware issue? If its software issue then how to solve it? Is hardware issue then what to do??
