What cause to psu cable to melt

Ultimately, because something happened that shouldn't have happens, it was a bad unit, or both.
I am not an electrician, but I think it is caused by too much current on the wire (or a short circuit)
Oh, you should probably never use this PSU again and hope it didn't take any more hardware with it.
