Go back to factory settings and try again.

Why do you have your core clock that high, that's insanely high even with extra voltage.

 

Start out at around +125 on core clock and stress test for 10 mins to see if it can sustain that. If it can increase the core clock by +10 and continue this until it fails the stress test. Once it does you have two options bring it back down to when it was stable or to start increasing the voltage to see if you can get better overclocks.

When overclocking, take it in baby steps. When I overclocked my 970, I increased the clocks by maybe 10-15MHz at a time, stressing each time using Heaven, and once it stopped boosting, I added additional voltage.
