Why does this happen? (Icon issues)

See how GPU-Z is just the default windows white block and not the normal icon? Why does this happen? Seems to happen intermittently on windows since Win 98.

theres some icon regen programs online, just run those (careful which one tho)

I assume it's having trouble finding the icon. I'm not 100% sure about it though, it's not really something I've ever bothered looking into.

The Icon losses track of the file path. Just happens for some reason.

it happens to me when i move the originall file so it losses its path

Right click icon>click properties>click customization> change icon>icon that it should be should be the only one to display>click it>click accept>done
