i think this is the wrong place to do such a survey because this isn't exactly the demographic your service is aimed at, you will probably get alot of no and unreasonably low values.

Just to point out, you should probably remove the name field. Most surveys/questionnaires will be anonymous in order to get accurate and truthful answers as it reduces the chances for social conformity.
