so this was done with a clean install. 

in the fallout.ini should be Documents>MyGames>Fallout 3> fallout 3.ini

edit it so it has these lines bUseThreadedAI=1 iNumHWThreads=2

 

Then when you launch fallout normally you get to this screen.

go to options. Turn off, Antialiasing, Anisotropic Filtering, Depth of Field, Transparency Multisampling and Water Displacement.

you also need the have the hdr 'screen effects' turned on.

 

after you have changed these setting you need to add FOSE.

 

In the Game folder rename the FalloutLauncher to FalloutLauncher.exe.backup. and the fose.exe to FalloutLauncher.exe

Add the enb files, then the d3d9.dll to the game directory. 

after this, my game opened fine and worked perfectly but would crash sometimes if I had Water displacement turned on.

it should work fine with mods as none of the files should be conflicting.

I also have the Xbox games stuff disabled through my registry, that might make some problems if its enabled, not sure.

in registry HKEY_CURRENT_USER\Software\Microsoft\GameBar

add "AllowAutoGameMode" and "AutoGameModeEnabled" as dwords with the value 0. it disables the game mode on the pc.

also try a fresh install if you havent
