I have Windows 7 and Fallout 3, works fine though I do have the occasional crash

It's not optimized in the sense that it doesn't run on quad core CPU's well.
 
I've had mixed experiences with Fallout 3 on Windows 7, sometimes it works without a hitch, then when I reinstall Windows for an upgrade, it crashes every 30 minutes.

Icon_information.png This will limit the game to 1 or 2 cores and prevent the engine bug from causing the game to freeze. Icon_plus.png The most common fix required for this game.

Follow this link for an in-depth guide 

  1. Go to http://www.mediafire.com/?flgl2nct1du0y1x and download d3d9.dll. When you download, make sure you click on the button which says 'Download (1.74 MB)'. There are usually ads on this page and I would take it under advisement NOT to click on anything else.
  2. Move the .dll file to your Fallout 3 folder. This can usually be found in C:\Program Files (x86)\Steam\steamapps\common\Fallout 3
  3. Open up the FALLOUT.ini file in: %USERPROFILE%\My Documents\My Games\Fallout3 using Notepad.
  4. Find the line: bUseThreadedAI=0
  5. Change it to: bUseThreadedAI=1
  6. Add another line after it and insert: iNumHWThreads=2
  7. If the game still crashes, try this line: iNumHWThreads=1

Played through the entire game without mods on Win 7 with no issues, at the time i used a GTX 480 if that makes any difference.

I run win8, it's the same. Sometimes it crashes, sometimes it doesn't. But after this fix it only crashes after about 3-4 hours, except in Operation: Anchorage where the further I got the more frequent the crashing got, getting as frequent as crashing every 10 minutes at the end. But once I got out of the simulation it ran fine (haven't got to any of the other DLC content yet).
