Knowing Apple, it could very well be a true 120Hz.
Yes, you will get downscaling via the YouTube app, alongside a better bitrate with the video. This really applies to any device that can sufficiently play YouTube but in the case of Apple products with, say, the most recent YouTube update and, again, say iOS 11, you can play up to 2160p video (most videos at that resolution get converted to VP9/Opus, so those will only allow up to 1440p as it uses the MP4 video and audio tracks) or 1080p60 video (explained above).
