SD card failing?

I have a SanDisk SD card I've been using with my camera for a while, and it's worked fine. However recently every time I plug it into a a windows PC, I get a little corner notification saying "There's a problem with this drive. Scan the drive now to fix it." It clearly actually is having problems, because at the same time Windows started saying that, the card occasionally disconnects then immediately reconnects while it's plugged in. This happens on multiple card readers and PCs. However when I tell windows to scan the drive, it doesn't find any errors, and I'm back to the same thing, still having issues. Is the card just screwed? It's less than a year old, so why would this happen?

Try a different card, see if that does anything.

Okay I think it may actually be an issue with the reader I'm using for my desktop, and also that card. Because on my laptop, the card in question gives the error message, but I'm not seeing the disconnects. My other card is giving me the disconnects but no message on my desktop, and neither issue on my laptop. So I guess the slot on my desktop and the card are both causing their own separate issues.
