Small cases generally blower.

With proper airflow, generally fan.

 

However you can get fan cooled cards that underperform, and blower units that OC like a beast.

Depends on the card, hoe many and the case. After spending quite a bit of time testing I prefer blower. Better in sli, less heat in the case and with single cards the temps are the same, fan speed depending. Most just let their card bake using the auto setting so it normally doesn't matter.
