How do people obtain ISOs from console games?

So I'm here downloading ISOs for emulators and I was wondering how do they get the actual ISO? You can't read it in a optical drive on PC right? So how do they do it?

Varies on what console we're talking about.

The PlayStation 2 is the only console that you can rip the game data without any modification and/or extra equipment DVD drive aside.

The GameCube and Wii use a Homebrew app on the Wii to rip the data.

Anything else varies.

Yeah, basically this. A modded PS1/2 Wii/WiiU/GC Xbox etc. can rip it's games to a Hard Drive, and then some kind of PC program copies them to PC. 

although, most of these programs also rip game discs onto PC directly either with a special optical drive or with 1337 h4x0rs
