How Do I know if I have a faulty R9 280x?

My R9 280x is being a bitch for these couple of months now:

1)sometimes the fans would stop spinning

2)Thermals reach up to 99C if fans stop spinning.(which causes it to throttle and be stuck at 300 Clock and 150 Memory untill I restart the system)

3)To keep it stable I need my clocks to be 890 On the core and 1250 on the memory,What do I do?

    Flatron L1960TR , MAG MS776I , ViewSonic VA903m ( all of them are 1280x1024,dad found em) and a Fujicom FJ-32V TV
  • Cooling
    1 200M front fan, and unknowen CPU tower(low profile)
  • Keyboard
    HP QY776AT
  • Mouse
    LUOM G10
  • Sound
    Jamo E 4 CEN, Jamo E470 and Jamo E410 with a KENWOOD S505D

Could it be a driver issue? Or Windows? Try installing MSI Afterburner and making a custom fan curve?

I did,my curve on Afterburner is 100% all the time.

and the fans wont spin.

It could be a driver or OS issue, but it could also be a GPU issue. Do you have someones PC you can test the GPU in? If it works there, then it's not the GPU. :)
