As the title say ....I've read about this but didnt actually believed it until recently when i got gta v.
It was crushing all the time i start searching about this error and the most common way to fix this it was underclocking your gpu.

And yes it worked O_o...What is your experience between your oced gpu and gta v???

what hardware are you running on, perhaps by under-clocking your GPU you were alleviating a bottleneck?

I have had that error a few times too. And it reduced after I ran the game as Administrator. Now I do not get that error anymore, or really rarely, like once every 3 weeks or so. So set your game to always run as Admin and hit us with results

After using 3D mark to alert me that my OC was a bit too high (flickering everywhere, the usual)

GTA V is really stable for me and our systems are almost identical, with my OC of 1367+60=1427mhz without fiddling with voltage. haven't been having any problems. which exact 970 is it?

err_gfx_d3d_init this error is very common, and isn't necessarily caused by you OC.

I get the same error once in a while if i'm watching youtube or some video on my secondary screen while playing.

I've also seen this error, when i OC'ed my 970 Strix.

Before a GTA 5 update, the error also came out of nowhere, everytime i had a cutscene in multiplayer.

My comp that is stable with overclocks using valley, furmark and prime95 yet crashes when I play GTA5, so now I use GTA to test overclocks, lol. Also all liquid cooled so it wasn't a temp problem.
