Constant GPU problems

Okay, in my sig are my specs.

A few weeks ago my card stopped working, it would not be recognized by my pc anymore, neither motherboard nor windows.

Sent it back for RMA and they say it works..
I was stumped but then I found out the GPU has a BIOS switch on it.

Flipped that switch and it worked again....

Now some days ago it again wouldn't work... The monitors again didn't get a signal. So it turned out to be a mobo setting which was put to use on board graphics (from last time so I could work on it)

Changed it to PCI-E and it worked again for some hours.

Then my middle monitor which is 144hz went to 60hz. Whenever I tried to set it to 144hz via windows it would cut off the signals to all monitors again, when I restarted the PC it was fine but the Middle monitor was at 60hz again.

I made an eyefinity group in CCC and disabled the group right away, and undid the 2% overclock on the GPU and everything worked agai and the Middle monitor could be set to 144hz again.

I put it on sleep mode, but when I turned the PC on again today from sleep mode.... Again the same problem and this time, no matter how many times I switch the Bios on the GPU.... The PC won't recognise the card, it refuses to. Thus I can't install CCC and the drivers.

I am completely stumped, could it be my motherboard?

Did you try safe mode to delete all the drivers and reinstall?

Was this with the new Omega drivers?
