Anyone else falling through the ground in Shadow of Mordor?

So uh.. I keep falling through the ground during missions which causes a mission fail... I have no clue whats causing it but its getting kinda annoying. 

 

I'm fighting one of the black hand captains and then right when i go to slash him I go straight through he ground and get a mission fail

Anyone else getting this?
