So my (quite old) 4890 is running at really hot temperatures. For some reason at idle I'm already at 80C, and under load it goes up to 100C. I've dusted my card regularly, but could there be another reason it's running so hot? (it does this even when it's not summer weather)

Have you tried re applying new thermal paste recently? If not do it, if you have already done so then i'd say you've used too much or too little.

Thermal paste? Fans working?

Thermal paste could be old and dried up, try re-applying.

That is probably the problem, since I haven't done anything with thermal paste since having this computer built. Are there any guides or posts that I can follow to do so?

Remove all the screws on the back of the card, wipe off old paste with isopropyl alcohol and coffee filters, apply new paste. Screw everything back together.
