if you are going for something custom milled, you likely want to align the screwholes properly and check that it doesnt interfiere with anything on the backside.
id suggest taking your own measurements when you get the card. that will also allow you to map out the socket properly. that or you will have to wait on teardowns from someone like GamersNexus.

if you mean the back of the socket, you will need to make room for the caps and SMDs, and prefferably not make it a heat trap. and the mounting must offcourse be accesible in some way.
i do also suggest making sure there is good spacing for the use of thermal pads if need be. something i dont know untill someone does a teardown.
not gonna give any great answer on the size and proportions, the main takeaway is that you should do this when you have the card in-hand
