Any fixes for GTA IV terrible optimization?

I know this game is old and was always a pain to run, no matter the rig, but does anyone know some fixes for the stuttering?

MSI B150 GAMING M3

MSI RX 480 4GB Gaming X

2x4 Corsair 2400 Mhz RAM

Kingston A400 240GB SSD

WD Blue 1Tb HDD

550W nJoy PSU 80+Bronze

Yeah, there are several unofficial patches for this game because it doesnt recognize all the VRAM of newer GPUs, but back when I played it I just looked an entire day for a good patch and then it worked very good, wait maybe I'll find one.
