Sides of bends bulging

title says it all. doing my bends and i keep getting the sides of my bends bulging out. not the inside or outside of the bend but the outside. anyone know what is causing this?

i am using a insert. this is why i'm not having any inner/outer radius problems.

You're bending the tube slightly too cold and also too narrow around the bend region, you need to heat up a larger region (i.e. more side-to-side)
