Case : Get either NZXT / Corsair / Cooler Master

Power supply : Cooler Master G650M, 650W

Here is what I would try first. (Before you send it back)
 
1) Try a different USB port. - For example if it's plugged into the front try the motherboard directly.
2) Uninstall any software or drivers and then re-install them.
 
If the problem persists after this, try the USB device on a different computer (If you have a second one).
If all of the above steps fail, I would send it back or get in touch with your supplier about the device.

Things you should consider when picking PSU.
1. Brand (Seasonic, Corsair, EVGA, Cooler Master)
2. Minimal 80+Bronze Efficiency
3. 500~600 wattage is the sweet spot for single GPU config

Steps :
1. connect ssd on sata_1, connect hdd on sata_2, plug in usb flash disk.
2. enter bios, set boot sequence to usb flash disk.
3. do fresh install on ssd via usb. (do format your hdd now and create a new partition / don't format your hdd if you want to back up something)
4. install the all drivers and other utilities (i suggest using ninite.com for utilities works like charm)
5. done
