Screen Pixelated

Hi Guys having this issue still screen keeps becoming pixelated after I wake my screen up, I have 2 screens and the other is fine when I wake it up just my main screen and even after changing a setting or just turning off the screen the whole pc crashes so need to restart, once restarted pc is working fine, I'm not sure what the issue is possible a doggy drive update via nvidia, my gpu is a 980ti and its not overheating checked temps under load and its peeking at like 45 or 50 degrees, see the image for the issue.
