Fast sync smoothness with higher than display framerates

If I understand correctly, running a game at say - 200fps even on a 60hz monitor SHOULD make the game feel more responsive and smooth, as there is less time between a frame being rendered and it being displayed on screen.

 

But does using the Nvidia control panel setting "Fast sync" which ditches frames that wont be shown on screen, remove this extra responsiveness from running games at a higher framerate?

 

Does Fast sync pick a random frame to display per refresh, or the one most recently rendered?

Fast Sync picks the most recently rendered one. It's a form of Triple Buffering

I never really noticed a difference between 200fps and 60 fps on my 60hz monitor. That's why I always have Vsync enabled.

Fast sync just uses the most recently fully rendered frame. When fps is below the refresh rate it has no advantages over v sync but at higher fps you get power input lag. It's not gonna improve smoothness since you're still displaying the same fps but as opposed to v sync, which will cap your fps to the monitor refresh rate, fast sync let's it go however high it wants leading to lower input lag.
