set target temp higher and the fan speed higher to keep it from thermal throttling

Whats the target temp set to then? Set it as high as it can go then set the fan to 100%

Becuase it looks like it's trying to purposely thermal throttle a temp that's set far too low.

target temp 91. power limit 139% . core voltage +87mV.

card is not thermal throttling. before the problem came about the  card was stable at 75 degrees which is normal

go nvidia driver set enery mode on max performance and also do that in windows settings. Skip the voltage set it to +0 and try it again. Whats ur core oc?

I don't see any issue at all.. That little fluctuation at full load, on the second thread, in that screen(GPU Usage %) looks pretty normal to me. Even my card fluctuates when going 100% from 1911 to 1899 and back to 1911 for some times.

try to set an offset of 100 core and see what happens

same thing happens just the core clock is 100mhz higher. i also tried to force a constant voltage

Maybe try rolling back your Nvidia driver.

Sometimes driver updates break stuff and cause big frame drops.

tried that still no results

So, wait...

by under load do you mean gaming or under a stress test?

It's normal for the GPU clock to drop down inbetween loading screens in games for example.

Run a constant stress test.

Like unigine superposition.. don't hit the benchmark button.. just let it run on the extreme preset..

Now watch your graphs while it's going and make sure it's keeping the clock rate up while actually showing scenes.
