Once every 3 years, really it makes no difference in 99% of the times, system can work fully even with those "registry leftovers" as they are tiny tiny meaningless bits of data.

A couple of times I've had driver crashes and bad installs/uninstalls that needed a registry cleaning, You only need to do it when there is a problem, it's not really something that should be part of a routine maintenance cycle, that said I do it monthly as I change drivers a lot.
