GTA V is very CPU intensive and if your CPU is pinned at 100% the only thing you can do to get more FPS with that CPU is to overclock it, but I doubt that it will give you any significant improvement.

 

EDIT:

Yes, there is possibility that some settings put more load on CPU at low and more on GPU at higher, so play with it a bit.

I have found luck changing the settings and balance between my cpu and gpu. Games ive played have given me the option within my EVGA OCX software to decide what does what amount of load and how fast it runs. I currently Use a FX-8320 with a 1050ti EVGA ssc. I play GTA 5 at 60+ FPS.. CPU usage at 67%, and gpu usage at 45%. Mind you my CPU is stable at 4.4 Ghz, and my Card is stock at 1.4 Ghz Turbo.

this is definitely not a GPU problem you are having. time to upgrade to a newer cpu

I play on lowest settings 1080p, only 30 fps in city. gpu usage 30% cpu 100%... but i see others with q8400 50+fps...

 

Should I put settings higher than low.. 30 fps is really bad...

Your CPU is bottlenecking your GPU.
