Annoying flash drive

Hello everyone, My flash drive is (or was) 16GB but up until recently hasn't been a consistent size at all, any idea on how i could fix it or is it dead? 

(and yes, i've tried it on multiple other pc's and getting the same result)

Check to see if there are any partitions that are on the same flash drive that are inaccessible
