You can try downloading/installing CrystalDiskInfo which will give you all the SMART information from the drive. Does the system freeze while running programs within Safe Mode?

I thought if you ran it in "safe mode" no programs typically appear? or would it be different since my OS is on a separate drive?

You can definitely run programs while in Safe Mode, regardless of their location.

As the guys suggested, you should check the drive's S.M.A.R.T. status and see what the raw values tell you. If you can't read them feel free to post them here and we will help you out! 

 

What is the brand and the model of the drive? You should be able to get a diagnostic tool from the manufacturer's website too. 

 

When you open Task Manager does anything hit 100% usage when the freezes occur?
