Update: I still have not been able to try another psu, but I hear the fan turning on and off repeatedly in idle. It should only turn on at 40% load which it is definitely not hitting.

Red flag. I'm not that experienced with Psu's but (total speculation) I think it's some sort of surge happening, or too much power being drawn.
