Windows 8.1 C Drive 100% Active Time

So my C Drive is running at 100% active time with no transfer rates and a near zero or zero cpu usage. Tried disabling the normal windows processes and only 9% of my HD is fragged. Going to run a transfer right now in case its a mechanical problem but I'm open to any suggestions

What processes are using the most of your disk?

Is it an HDD or an SSD?

 

This is a common issue, I had it myself. Can't remember how I fixed it at the moment :P

System is hovering at about .4% w ith some service host around the same. When the computer boots two things happen;

 

on the processes tab it shows the disk stuck at 33%

but on the performance tab it spikes directly to 100$ and doesnt move

 

super weird
