AMD Opteron Prototype Cabinet, and sorry, they are very difficult to come by as they were used for demonstration. I am pretty sure there was only 12 but some were rumored to be sold for the scrap metal.

I am assuming it is a custom made case and not a commercially made one.

Keyword is 'prototype'. It looks like these things are pretty much unattainable now.
