Which reading should I trust?

HWMonitor says 1.276v and CPU-Z says 1.424v. Which one is more accurate?

Samsung 850 EVO 240 GB 

138 is a good number.

I usually trust CPU-Z but use a few other programs to take some readings since you seem to have a fairly large discrepancy and 1.424V would be close to the absolute max of the core voltage which isn't good if that was what your putting into your chip.

Im pretty sure that I put it to auto or around 1.2v

Im going into the BIOS to check... which i should had done..

Yeah better look into that, don't leave it on auto when dialing in an overclock since it just scales voltage making it go pretty high.

What voltage should I use for 4.5ghz? 1.2, 1.225, 1.235, 1.25, 1.275 and 1.3v doesn't boot up.

It depends on your chip, I assume the 3770k from the sig, since every chip is slightly different on how well it overclocks there isn't a specific voltage but a good rule of thumb is around a max of 1.3V on air and 1.35V on watercooling or large air coolers. 

 

Start off at base clock and slowly bring up the multiplier a bit at a time and go for a quick 10-20min stress test to see if that voltage is stable, once it fails you know to either dial back the multipler or up the voltage a bit to see where it's good again and stress test each time.
