1.45v, not the most desirable setting.  It might be the reason for the now unstable clock.  That is way more juice than OC'ers here would recommend for a 24/7 OC.  <<I might be wrong

 

Normally 1.3v or less is the 24/7 voltage recommendation.

I would revert to stock settings, and then test it.  If it unstable, contact Intel and try to get a new CPU.  They do exchange CPUs that are damaged.

 

If it is stable, then do the overclocking again.  BUT... do not throw 1.45v at the thing for a 24/7 overclock.

1)Your OC never was stable and you just crashed

-or-

2) You are using too much voltage , which causes the transistors to deteriorate , and require more voltage to operate
