Some unknown crazy 600w worthless PSU.

(its not even on the internet)

This problem can also be caused by a bad quality PSU. In general you rather want a good quality PSU.

I recommend trying http://www.guru3d.com/files-details/display-driver-uninstaller-download.html

then;

Right-click your computer name in Device Manager and click Scan for hardware changes. Device manager scans your system and automatically reinstalls your device.

Right-click the device and click Properties. On the General tab of the device Properties dialog box, in Device status, you should see the message This device is working properly.

It's either drivers or a dead GPU i'm afraid.. have you overclocked your GTX 960 ever?

and I never OC my GPU

Have you tried taking your GPU out of your computer, I know it might not make sense but I had a similar issue and when I pulled my GPU out and then put it back in it fixed it self. It's worth a shot, hope it helps.

Try that. Unplug your whole videocard get the dust out of the fans and get it back in. Plug everything in (even in your monitor) and see if it works. Make sure you use the right DVI-cable to your monitor and see if it works.

What cable are you using btw? DVI or VGA?

I doubt it's the cable as you get an error from the gpu itself.

Go through the troubleshooting steps in on this website. If nothing fixes the problem, its a hardware problem. Probably not a PSU, but more a GPU or motherboard issue. Or your PSU is bad with protecting your sustem from voltage discharge.

Error code 43 is a generic message that typically indicates a problem with the hardware or software in Windows operating system.  This error is often reported in Windows Device Manager. If you encounter this error with your graphics card, here are some troubleshooting ideas that may help fix it.  If you continue to have problem, please contact your manufacturer.

1)  Update your graphics card driver from NVIDIA.COM web site - CHECK

2)  Make sure your system is updated with the latest SBIOS and Windows updates

3)  Check to make sure your system power supply meets the minimum recommended power requirement for your card

4)  If your card needs additional power,  make sure the PCIe supplemental power connector is plugged into the card

5)  Replace the graphics card as it may be defective

Do number 5 as last resort.

I literally put a website on troubleshooting methods, and I hope you read this entire situation.

Because almost all things you mentioned have been tested.

Well, when you replace your Graphics card, be sure to add a better quality power supply as well, to prevent this from ever happening again.

The power supply must have send a power surge of electricity that f'd up your GPU. Sorry about your luck mate.
