USB boot drives are normally used for for installing an OS.

Other than that, you can also have a Linux live distro in one, and it will load up the OS into memory.

Here's how Linus explains it. Windows USB boot drives are only for installing and restoring. Linux can be either to install and restore or used as live sessions where any setting you change will be reset on boot. Some Linux distros can be used as portable OS' with USB drives.

I have a couple of distros of linux in my pocket now lol!

You load up the USB with the OS of your choice and then in the bios of the machine set it to boot from it. 

Also you can set it to have persistance meaning that the changes you make to the system will actually be saved to the drive itself! Its quite cool. (also good for getting into systems that youre not allowed to)
also USB boot drives are used for programs like KONBOOT (look it up its really cool)
