Screen goes black and PC's fans go to max speed when alt tabbing or changing resolutions in a game.

I have no idea what is happening. More often i notice my PC just goes dark on me when i'm playing a game. Forcing a restart but this issue is just getting out of hand.

Does anyone have any idea what the problem could be?
