i/o error on USB stick

I was trying to copy a file, wasn't worning so I tried to format it, didn't want to format so did diskpart clean, then tried to format it again and now i get i/o error when I plug itspacer.png

 

is it dead?

Could be a corrupt file system.

Then the drive should not have a drive letter...

Use Disk Management instead.
