get a stereo extension cable from your headphones/audio output of ps4, to your pc's line in jack, then enable line in, in the volume control application or the application that came with the sound card.

Example cable : https://www.amazon.com/dp/B00NO73Q84/

The more complex and expensive option is to get an audio mixer.

User two of the above cables to direct the audio output of both PC and PS4 into a mixer, then plug your headphones into the mixer.

You can control the volume of each input from the mixer, using knobs or sliders.

example of super cheap mixer : https://www.amazon.com/dp/B000KGYAYQ/

takes 4 inputs and does one output and you can control volume of each input.

The inputs are 1/4" TRS but there's cheap 3.5mm to 1/4" TRS adapters available : https://www.amazon.com/Cable-Matters-5-Pack-Plated-Adapter/dp/B00PIWB2SO/

You won't be able to send your headset's microphone to the PS4 if you do this.

I suggest you get a mixer to which all your audio goes to.

PC -> mixer channels 1&2

PS4 -> mixer channels 3&4

Yamaha MG series mixers are awesome quality and are very well priced.

Remember that you need TWO CHANNELS per device because you want stereo sound, not mono.

You have text printed by the jacks.

For 5.1 , you plug them in  line out (L_Out stereo left and right) , and the jacks to the left (RS - side speakers, CS - center and subwoofer)

For 7.1, you repurpose the line in connector, the extra speakers go in that jack above the pink line out jack

The mic (plug below pink line out) can be used for microphone.
