Audio in Headphones Sound like Earrape

I don't know when this started but a couple of weeks ago the audio from my headphones sounds like they decibel level was turned up to ear-rape levels. I made my computer so that the audio from to my headphones and speaker at the same time with setero mix. Please help I have torn my hair out of frustration.

have they always been hooked up the same way?  could you describe how you have them hooked up in more detail?
